**Plot Summary:**
Sarah, Juno, and Beth, three thrill-seeking friends, enjoy whitewater rafting together. Tragically, Sarah’s life takes a dark turn when she, her husband Paul, and their daughter Jessica are involved in a car accident caused by Paul’s distraction. Paul and Jessica do not survive, but Sarah is left to cope with the devastating loss.
A year later, Sarah, Juno, and Beth, along with friends Sam, Rebecca, and newcomer Holly, gather at a cabin in the Appalachian Mountains of North Carolina for a caving expedition. The group hikes to a mountain cave entrance and begins their descent. Inside the cave, Juno apologizes to Sarah for not being supportive after the accident, but Sarah remains emotionally distant. As they explore, a narrow passage collapses behind them, trapping them inside. Juno reveals that she led them into an uncharted cave system instead of the planned, well-explored one, making rescue unlikely. She admits her hope was to mend her fractured relationship with Sarah.
As the group continues searching for an exit, they find old climbing equipment and a cave painting hinting at a possible way out. However, Holly, believing she sees sunlight, runs ahead and falls, breaking her leg. While helping her, Sarah encounters a pale, humanoid creature drinking from a pool. The creature flees, but the group soon discovers a den filled with animal bones. They are suddenly attacked by the creatures, referred to as “crawlers.” Holly is killed when one of the creatures bites through her throat. Sarah falls down a hole and loses consciousness.
In the chaos, Juno tries to defend Holly’s body but accidentally kills Beth in panic, leaving her to die. Beth clutches Juno’s pendant as she pleads not to be abandoned, but Juno flees in shock.
The remaining survivors must now face the terrifying reality of their situation as they struggle to escape the cave and avoid the relentless crawlers.
