In February 2019, MGM acquired a spec script titled Samaritan, written by Bragi F. Schut, which was to be co-produced with Balboa Productions. Schut had initially developed the screenplay before adapting it into graphic novels. By September 2019, Julius Avery signed on as the director. Stallone was announced as the lead actor and producer in February 2020, with supporting roles going to Martin Starr, Moisés Arias, Dascha Polanco, Pilou Asbæk, Javon Walton, Jared Odrick, and Michael Aaron Milligan. Natacha Karam joined the cast in March 2020.
Production began in Atlanta in February 2020 but was paused on March 14 due to the COVID-19 pandemic. Filming resumed by October 8, 2020.
