The story begins with Sara being arrested for the murder of Christina Scofield Hampton during her wedding reception with Michael. Surveillance footage is altered to make it appear as if Sara shot Christina in cold blood, rather than in self-defense. Sara is sent to the Miami-Dade State Penitentiary, where female inmates are housed alongside jail detainees due to overcrowding. Gretchen Morgan, now imprisoned after events in “The Sunshine State,” observes from afar. Sara becomes a target of revenge by Miami-Dade correctional officers for her role in the Fox River escape, which led to the dismissal or suspension of several staff members.
In the men’s facility nearby, Krantz and Theodore “T-Bag” Bagwell are held. T-Bag assists Krantz, despite the latter’s claims of being unable to pay for contraband. Krantz offers Gretchen $100,000 to kill Sara, but when she delays, he places a bounty on Sara’s life. Sara is poisoned with strychnine but survives after receiving medical treatment. The prison doctor informs her that she will have only 30 minutes with her newborn before the child is taken away, advising her to view herself as a surrogate to avoid severe emotional distress.
Upon learning of the attacks through Sara’s lawyer, Michael appeals to the warden for her protection, but his request is denied. The warden also restricts Sara’s visitors to her lawyer alone. Realizing the danger, Michael decides to break Sara out of prison. He reunites with Lincoln Burrows and Fernando Sucre to plan the escape. As Lincoln and Sucre scout the prison, Gretchen notices their activity and demands to be included in any escape plan involving Sara.
