Inside Frank’s body, referred to as the “City of Frank,” a white blood cell named Osmosis “Ozzy” Jones works as an overenthusiastic officer in the Frank Police Department, which handles threats to the body.
Ozzy was demoted to patrol duty in the mouth after causing Frank to vomit on Shane’s teacher, Mrs. Boyd. The incident occurred when Frank ate a contaminated egg, leading to his humiliation, losing his job at a pea soup factory, and being banned from Shane’s school due to a restraining order filed by Mrs. Boyd.
Years later, Mayor Phlegmming, facing a tough re-election campaign, prioritizes junk food policies, ignoring their impact on Frank’s health. Frank consumes a boiled egg contaminated by a chimpanzee, introducing Thrax, a deadly virus known as “The Red Death,” into his system. Phlegmming signals Frank to take a cold pill, Drixenol “Drix” Koldreliff, which disinfects the throat but hides Thrax’s presence. Ozzy is reluctantly assigned to assist Drix in the investigation.
Thrax leads a gang of sweat germs and attacks Frank’s nose, nearly killing Drix before Ozzy saves him. The duo visits Chill, a flu vaccine and Ozzy’s informant, who directs them to Thrax’s hideout in a germ-infested nightclub inside a large zit on Frank’s forehead. Ozzy infiltrates Thrax’s gang and discovers his plan to masquerade as a common cold while using DNA knowledge to kill Frank within 48 hours.
When Ozzy’s cover is blown, Drix intervenes, sparking a fight that ends with the zit exploding. The pus lands on Mrs. Boyd’s lip during Frank’s attempt to apologize, ruining his chance at reconciliation. Mayor Phlegmming shuts down the investigation, fires Ozzy, and dismisses Drix, reminding him that his services are temporary.
