**Storyline:**
Machete Cortez teams up with agent Sartana Rivera to apprehend corrupt military officials involved in a weapons deal with a Mexican drug cartel. The gangsters eliminate the military team, but they are subsequently killed by a group of masked men led by a luchador-masked figure who also kills Sartana. Machete is captured, hanged by corrupt Sheriff Doakes and Deputy Clebourne, but miraculously survives. He receives a call from U.S. President Rathcock, who offers to clear Machete’s criminal record and grant him citizenship in exchange for stopping Marcos Mendez, a deranged psychopath who threatens to launch a nuclear missile at Washington, D.C. unless the U.S. government halts the cartels and the corrupt Mexican government.
Machete heads to San Antonio, where he meets Blanca Vasquez, his handler and a beauty contestant. In Acapulco, he searches for Cereza, who can lead him to Mendez, but faces an attack from Desdemona, a brothel madam and Cereza’s mother. Cereza, beloved by Mendez, warns Machete about Mendez’s split personality. As they journey to Mendez’s base, Mendez’s enforcer Zaror receives orders and kills Cereza.
Machete discovers that Mendez has wired the missile’s launch device to his own heart, ensuring that if he dies, the missile will fire. Mendez activates a 24-hour timer after killing the device’s designer. Machete kills Zaror and resolves to escort Mendez to the U.S. to disarm the missile. Along the way, Mendez reveals his backstory as a self-proclaimed secret agent who was betrayed by corrupt superiors, leading to the torture and murder of his family, which resulted in his split personality.
