In 517 A.D., the malevolent Blood Queen Vivienne Nimue unleashed a plague upon England. King Arthur, with the help of Ganeida, a member of Nimue’s coven, stopped her. Using Excalibur, Arthur dismembered Nimue and scattered her remains across England.
In present-day Tijuana, Mexico, paranormal investigator Hellboy accidentally kills a missing agent, Esteban Ruiz, who had been turned into a vampire. Before dying, Ruiz warns of an impending apocalypse. Hellboy returns to the B.P.R.D. base in Colorado, where his adoptive father and B.P.R.D. leader, Trevor Bruttenholm, assigns him to assist the Osiris Club in hunting three giants in Great Britain. Lady Hatton, the club’s seer, reveals that Bruttenholm was meant to kill Hellboy when he first appeared through the Nazis’ Project Ragna Rok but chose to raise him instead.
Meanwhile, Gruagach, a pig-like fairy seeking revenge against Hellboy, is instructed by the witch Baba Yaga to collect Nimue’s remains. During the hunt, Hellboy is betrayed by the hunters and nearly killed, but he survives and defeats the giants. Exhausted, he is rescued by Alice Monaghan, a medium he once saved from fairies as a child.
Bruttenholm sends a team to retrieve Hellboy and introduces him to M11 agent Ben Daimio. They learn that Nimue’s remains have been taken, with the last piece stored at the Osiris Club. Upon arriving, they find the club slaughtered. Alice channels Lady Hatton’s spirit, revealing that Nimue seeks Hellboy to bring about the apocalypse. Nimue’s arm is stolen by Gruagach, and she manipulates Hellboy’s frustrations to distract him, allowing Gruagach to escape.
Hellboy explains that Gruagach is a changeling who once replaced baby Alice. Hellboy branded him with iron and forced him to return Alice, which fueled Gruagach’s hatred for taking away his chance to become human.
