**Plot Summary:**
A young couple, Aki and Kazuo, are abducted during their first date and wake up chained in a plastic-covered basement. Without explanation, a sadistic captor subjects them to extreme torture and degradation. Kazuo is stabbed with a screwdriver, his tongue is sliced, and nails are driven into his scrotum. Both are sexually assaulted, forced to witness each other’s suffering. The captor occasionally pauses to treat their wounds, ensuring they survive longer to endure further torment.
The torture intensifies as their fingers are severed, Kazuo’s eye is gouged out, and Aki’s nipples and arm are removed. It becomes clear the captor derives sexual pleasure from their pain. Eventually, he castrates Kazuo, claiming he no longer needs their “services.”
The couple is moved to a clean, hospital-like room where their wounds are tended to. The captor’s refined manners, medical expertise, and taste for classical music, fine wine, and expensive clothing suggest he is a wealthy, possibly respected surgeon. Despite his polished appearance, a foul odor hints at his hidden depravity.
After days of recovery, the captor tells them he will release them, turn himself in, and give them his fortune as compensation. Aki and Kazuo vow to support each other once free. However, in a cruel twist, they are returned to the basement and shackled again, their hopes of freedom shattered.
