**Storyline:**
Jerry Hartfield, the producer of the reality TV show *Grave Encounters*, introduces the series, which was canceled after five episodes due to the mysterious disappearance of its crew. The film presents recovered footage from the sixth and final episode. The crew, led by ghost hunter Lance Preston, includes occult specialist Sasha Parker, surveillance operator Matt White, cameraman T.C. Gibson, and fake medium Houston Grey.
The team is invited to investigate the abandoned Collingwood Psychiatric Hospital, known for years of reported paranormal activity. During a daytime tour, the warden shares the hospital’s dark history, including the unethical experiments and lobotomies performed by Dr. Arthur Friedkin, who was later killed by his patients.
For their investigation, the crew locks themselves inside the hospital overnight. Initially, nothing unusual happens until a door slams shut behind T.C. As they attempt to communicate with the entities, the situation escalates into hostility.
When the warden fails to return to unlock the hospital, the crew tries to leave but finds that the building traps them. Every exit leads to another hallway, and time seems to stand still outside. They encounter a girl whose face distorts horrifically and witness Houston’s death by an invisible force. The crew discovers they are wearing hospital ID bracelets with their names.
Eventually, they find Matt, now insane and dressed in a hospital gown. He claims that the only way to escape is to be “cured” by the hospital’s spirits.
