Set in the 19th century at Blackwood Manor in Providence County, Rhode Island, the story begins with Lord Blackwood, a renowned wildlife painter, who brutally murders his housekeeper in the basement. He extracts her teeth and his own, offering them to mysterious creatures residing in an ash pit within an old fireplace. However, the creatures reject his offering, demanding only the teeth of children. Desperate to save his kidnapped son, Blackwood pleads with the creatures but is ultimately dragged into the pit.

In the present day, 8-year-old Sally Hurst arrives at Blackwood Manor to live with her father, Alex, and his girlfriend, Kim, who are restoring the manor to sell it. Struggling with her mother’s decision to leave her in Alex’s care and the effects of excessive Adderall, Sally feels isolated. The creatures are awakened by a melody from her nightlight. The following day, Sally hears their voices calling her name and discovers a sealed fireplace with the words “BE AFRAID” carved above it in runes.

Curious, Sally unseals the fireplace and finds a tooth belonging to the old housekeeper. The creatures soon reveal their hostile nature, stealing Alex’s razor and destroying Kim’s clothes. Alex accuses Sally of the mischief but discovers a 19th-century silver coin in her possession, which she found under her pillow after the tooth vanished.

Sally ventures into the basement to confront the creatures but is interrupted by Mr. Harris, a worker, who sends her away and attempts to seal the fireplace. The creatures viciously attack Harris, leaving him severely injured and hospitalized.

Kim visits Harris, who advises her to search for Lord Blackwood’s unpublished artwork at the local library. The librarian identifies the creatures in the artwork as tooth fairies, explaining that they can transform humans into their kind. Sally is attacked again by the creatures, this time led by Lord Blackwood, who has become one of them.
